ACTIVITY FORM - If you are a guest and wish to schedule an activity on the church’s campus please use this form.  Make sure to fill out the form completely and turn it in to the church office. Be sure to read through our policies concerning activiy forms. If you are a church member login to Access ACS, click on "calendar," and then click the "add event" button near the top right to submit a digital activity form to the church office. Detailed instructions are provided here.

WEDDING RESERVATION FORM - If you wish to schedule a wedding on the church’s campus please use this form.  After the download please print this form and fill it out completely.  Once completed please drop the form off at the church’s main office.  Copies of this form may also be found in the church’s office. Please be sure to read through our wedding policies before scheduling.

YOUTH MEDICAL RELEASE FORM - Certain youth functions require a medical release form.  After the download please print this form and fill it out completely.  Once completed please drop the form off at the church’s main office giving special attention to Rob Cassell as the recipient of the form.
